Complementary care coordination ensures maximum resources are being afforded to our clients & sensible steps are being taken to ensure the plan of care is sustainable. This is evaluated not only from a care aspect, but from a financial perspective too.
Includes evaluation of the environment in which a client receives care-through home safety assessments & recommendationS for additional community health resources.
